There's this great thing called interception nowadays. The Tsar Bomb is slow, heavy and the planes that are needed to carry it aren't exactly the least noticeable.

Yes it's a nuke, but getting it towards a target in a modern world is pretty much impossible. Even the Russians themselves said: the bomb is too big to use realistically.

Yep, true, it could be intercepted. And yes, it is hard to use, but who cares? I mean, this world is fake, so I mean, it could work.
