I'd say it's just poorly maintained buildings. I've never heard of bed bugs being much of a problem for any community that isn't low-income, for the US at least.

I say big cities because in places like NYC, things are so closely packed they just move around. In more spread out cities there are large poor neighborhoods for them to hide in, like Detroit and Chicago. That's why I say big cities, because most of them are in big cities, except for individual buildings that aren't maintained properly in smaller towns and rural areas.
